fix: replace fatalError with proper error handling
Replaced all fatalError calls in superEncoder/superDecoder methods with proper error handling following established patterns. Changes: - FormEncoder: Created ThrowingStubEncoder to handle superEncoder() methods - Cannot throw per protocol, so returns stub that throws on use - Provides clear error messages about unsupported operations - FormDecoder: Replaced fatalError with proper throw in superDecoder() - All 218 tests pass This ensures no crashes and follows the pattern of never using fatalError or precondition.
